Magic the Gathering damage question...?

A lot of cards in MTG have text that indicates "damage to creature, damage to player, and damage to opponent". The damage to creature and damage to player commands are straightforward, but when it reads "damage to opponent" (like in warren instigator) does it include damage in combat involving creatures as well as damage to the opponent's life? Thanks.
